What to Bring to Your Appointment Patient Forms Insurance and Payment Information Hospital Affiliates Privacy Practices We realize that our patients' time is valuable, and we make every effort to see patients at their scheduled appointment time. We ask that patients provide as much advance notice as possible when running late for an appointment or when cancelling. With this advance notice of cancellation, the time can be scheduled for another patient. What to Bring to Your Appointment If you are a first-time patient, please plan on arriving early to complete a patient registration form. Please bring the following items to the appointment:
We are pleased to provide you the opportunity to print the patient forms below. Please complete them and then fax them to our office (563.355.7396) so that all the paperwork will be already entered when you arrive for your office visit. If you are not able to fax the forms, please mail them to our office or bring them in at the initial office visit.
[information on accessing and printing forms] Insurance and Payment Information Most insurance plans are accepted. Our office handles all of the filing and paperwork for you. We also do all of the secondary billing for you as well, if you have secondary insurance coverage. We accept assignment on all Medicare and Medicaid (title 19) claims. We will contact your insurance company for you if there is a question whether a particular service or procedure may or may not be covered. Area Wide Foot and Ankle Center has two different hospital affiliates in the Quad Cities Area.
